2. You need to fill out the online application form.

PAYMENT PROCESS

3. Pay your one-time lifetime membership fee and send your payments thru Bank Deposits, Online bank Transfer or Remittance.

After mag-deposit sa bank account ay paki-picturan or i-screenshot ang proof of payment at isend ulit ito sa email address ko para ma-print at ma-attached sa Kaiser Application Form.
